REGISTRATION

Online Registration for the 2012 season will be available in the spring.

Note the following:

Registration Fee is due at time of registration, non-refundable.  Checks should be made out to "ORYF INC."

1. You must complete and submit a Pop Warner Medical Form.  Section II of the form must be filled out by a medical professional or you must attach your medical professional's office form to this form.  The medical professional must sign and date the form on or after January 1 of the current year, if you do not understand what this requirement means, contact Lisa Flynn at 508-763-4475.

2. You must complete and submit a Pop Warner Player Contract Form.

3. You must provide a copy of every page, including the cover page, of your final report card for the 2010-2011 academic year.  If you register prior to issuance of final report card please remember to submit the final report card when available.

4. If you are a new participant you must provide BOTH an original birth certificate and a copy of the birth certficate (passports and military ID's are also acceptable, contact Tom Flynn at 508-863-0518 for details).  Just submitting a copy is not acceptable.  If you want the original returned to you, you must provide a self-addressed stamped envelope.

Please remember that if you or someone you know needs assistance with the registration fee please contact Tom Flynn.  ORYF offers partial and full fee waivers for children, including mutli-child discounts for financially needy families.  NO CHILD WILL EVER BE DENIED ACCESS TO ORYF BASED ON FINANCIAL CONSTRAINTS.

ONCE YOU HAVE YOUR REGISTRATION FORM (PLAYER CONTRACT) FILLED OUT, SEND IT IN IMMEDIATELY.  YOU CAN SEND IN YOUR MEDICAL FORM AND REPORT CARD WHEN AVAILABLE.

REMEMBER, REGISTRATION IS ON A FIRST COME, FIRST SERVED BASIS.
